Description
The Care Plan / Reminders / Tasks Section includes any specific planned or scheduled
tests, procedures or regimens of care to be performed by the physician or agreed upon
by the patient. This section includes reminders and tasks to be performed by the care
provider team. Examples of tasks/reminders include laboratory, imaging, consult, communication
actions that need to be performed. These form the basis of Chronic Disease Management
and proactive care. This section could also incorporate Targets/Goals of care that
are both standard population based (PAP smears,
mammograms) and individualized (diabetic testing in diabetic patients).
