The Record ID Link template is a generic template that is referenced by various section
or entry templates to enable (1) trouble shooting, particularly in conversation processes,
by enabling the inclusion of a reference to an original record in a source system,
and (2) the consistent communication of a link to
another record in the patient’s medical record, or within the document as required.
For example, a
prescription may link to a previous prescription or a diagnosis may link to a problem
list record.
The Record ID Link template supports a single
observation/id
element with a Record ID including the assigning authority of the identifier.
Record ID
A record identifier element may be expresses using the root attribute or the combination
of root/extension attributes. If the root attribute method is used, the source system
shall generate and assign a unique identifier in the UUID/GUID format.
If the source system uses root and extension attributes to identify records, the combination
of both attributes shall form a unique identifier within the document instance.
The source system
SHALL
preserve assigned record identifiers in the database to support an ability to troubleshoot
issues.
Caveat
It is understood that the record ID should likely be communicated through a
Value
attribute since the
ID
attribute, strictly speaking, refers to the identification of the observation used
to communicate the record Identifier. However, since this has been modelled in the
current fashion for several releases, this is being maintained to protect current
implementations.
