Description |
The Medication Identification List template defines the structure to identify the
specific drug and drug strength that is the subject of the Medication List record
as defined in the Medication Observation template.
CDA Extensions
This template includes the following CDA Extensions:

Drug Description: The Drug Description (LabeledDrug/e2e:desc)has been added to
the schema to support a free form textual description of the drug. This is usually
only populated for custom compounds, providing instructions on the composition and
creation of the compound.

Drug Form: The Drug Form (LabeledDrug/e2e:formCode) has been added to the schema
communicate the administration form of the drug (e.g. tablet, capsule, powder, etc.).

Drug Ingredient: Frequently the drug strength is assumed to be included in or implied
by the drug code itself. For example, there is a specific DIN code for TYLENOL 325
mg tablet (00723894). However, the specification supports the communication of the
drug strength attribute as a separate element to accommodate historical or patient
reported data that may not be coded or may not have the strength pre-coordinated in
the drug code or implied through the drug name.

Generic Name
The Generic Name is used to communicate the generic ingredients that are included
in the drug. This is included as the <translation> component of the Drug Code element
which may repeat.
In some cases there will be multiple generic named ingredients within the drug; for
example, Tylenol 3 has acetaminophen 300mg and codeine 30mg and caffeine as generic
ingredients. This element is a string format and there are no requirements for the
display of the ingredient name, strength and units of measure; however, it is strongly
recommended that the element repeat with each ingredient included in its own repetition.
Having this recorded as a single text string causes difficulties for calculating defined
daily dose when a medication
formulation consists of a combination of different drugs.
This field may be absent if a DIN code is provided as the receiving system may use
the DIN code to identify the component generic ingredients.
Drug Description
The Drug Description is a free form textual description of the drug. This usually
is only populated for custom compounds, providing instructions on the composition
and creation of the compound.
This field may also be used to provide more detailed and supplemental information
regarding the Drug; however, dispensing and administration instructions should not
be included here as there are specific instruction fields for that information.
